The answer, with the useful detail.

A short private setup record can save time after a device reset. Record the television model, player name, publisher, app version and legitimate support route. Store actual passwords and account-bearing playlist links in an appropriate private credential store rather than in a note displayed beside the TV. A useful record explains how to reconstruct the setup, not merely which button opens it today. Include the installation route and source format while keeping the actual secrets in a separate controlled location.

Record identity accurately

Several players can have similar names and icons. Include the publisher or package identity so a later reinstall does not accidentally select a different app. Record whether the app runs on the television itself or an HDMI box, including that device's full model and operating system. Add the official publisher link and installed edition. A note containing only Smart IPTV or a coloured icon can point to several unrelated applications, making a later reinstall harder and increasing the chance of entering credentials into the wrong software.

Separate public and private details

Device model and app version are useful troubleshooting facts. Passwords, QR codes and provider URLs with credentials need more careful handling and should not be included in a shareable checklist. Keep account email addresses, passwords, full playlist links and restoration codes out of a note intended for general support. Mark where the legitimate recovery information is held without copying it into the public checklist. If a configuration backup contains secrets, protect it accordingly. A masked screenshot is not necessarily a recoverable password record and should not replace the original issued details.

Update the record when the setup changes

If you replace the player or move to another device, update the record at that time. An old screenshot of an account page is a poor substitute for a concise, current setup note. Revise the model, edition, version and installation route when changing the setup. Record whether the player imports a local file or reads it from USB so a later move does not omit an essential dependency. Test that the recovery location remains accessible to the responsible household member; an accurate note is only useful if its separate account recovery information can actually be retrieved.

An example to work through

A note saying “Google TV, VLC by VideoLAN, current version, provider support number” is useful without exposing a provider password. Add whether VLC runs on the TV or an external box and where private recovery information is kept. That lets a household member reconstruct the correct installation route without exposing the account itself.

Questions about this topic

Should I print a full credential-bearing playlist URL?

Avoid leaving access details visible in a shared room.

Is the player name alone always enough?

No. Include the publisher because similarly named applications can be unrelated.
